sa palagay ko po wala namang prescribed format yong CAF basta nakalagay sa CAF kung anong basis nya ng funding at magkano yong amount at pinaka importante po is pirmado ng Accountant based on PD 1445. Usually po yong for public bidding nilalagyan ng CAF pero yong maliliit lang po ok na mag sign sa PR na may funds available.
