How to re-order/purchase MDS Check Booklets from DBP, LBP, and PVB?

The Bureau of the Treasury (BTR) provides guidelines on how to re-order/purchase Modified Disbursement System (MDS) check booklets from any of the authorized government servicing banks, i.e., Landbank of the Philippines (LBP), Development Bank of the Philippines (DBP) and Philippine Veterans Bank (PVB).

To re-order or purchase MDS check booklets, just submit a letter request using the forms below and submit the same to the nearest BTR Regional/Provincial Office.

Request Forms for the Re-Order/Purchase of MDS Check Booklets

Once the request was assessed by the BTR Regional/Provincial Office, they will send a confirmation letter containing the number of check booklets which can be re-ordered/purchase by the agency, copy furnished the government servicing bank concerned. A sample format of the confirmation letter is shown below.

Note: These procedures are applicable only to government agencies which maintain MDS Sub-accounts with the authorized government servicing banks.

MDS Sub-accounts are bank accounts maintained by government agencies which funds are received from the Department of Budget and Management through Notice of Cash Allocation (NCA).

To order non-MDS check booklets, follow the procedures prescribed by the bank concerned.
